Participation Waiver, Release of Liability, Assumption of Risk & Photo Consent
Please read this agreement carefully before entering the play areas. The museum is designed to inspire imagination, creativity, exploration and hands-on learning. Although the museum works to provide a safe environment, interactive exhibits and play activities involve inherent risks. By submitting the digital form, the parent or guardian acknowledges and accepts those risks.
1. Assumption of Risk
Participation in museum exhibits, climbing structures, sensory experiences, STEAM activities, imaginative play, water play, art, gross-motor activity, outdoor play and other attractions involves risks that cannot be completely eliminated.
Risks may include slips, trips and falls; minor cuts, bruises, scrapes or bumps; collisions with other guests; falls from play equipment; allergic reactions; exposure to common childhood illnesses; injury resulting from another guest’s actions; equipment failure despite reasonable maintenance; and risks associated with physical activity. The signer voluntarily assumes all risks associated with participation.
2. Parent/Guardian Supervision
The City of Great Discovery Children’s Museum is not a drop-off facility.
- Children must be supervised by a responsible adult at all times.
- The parent or guardian remains solely responsible for the child’s behavior and safety.
- Museum employees do not provide one-on-one supervision.
- Young children must be accompanied throughout the museum.
3. Medical Authorization
In an emergency, the signer authorizes museum staff to contact emergency medical services by calling 911. The museum does not provide medical treatment. Emergency transportation and medical expenses remain the signer’s responsibility.
4. Play Rules
- No rough or aggressive play.
- No climbing in areas not intended for climbing.
- No throwing toys or equipment.
- Walk in designated walking areas.
- Be kind and respectful to others.
- Use exhibits as intended and follow staff instructions.
Failure to follow museum rules may result in being asked to leave without a refund.
5. Personal Belongings
The museum is not responsible for lost, stolen or damaged personal items, including phones, jewelry, clothing, strollers, diapers, toys, gifts or other belongings.
6. Liability Release
To the fullest extent allowed by law, the signer agrees not to hold the City of Great Discovery Children’s Museum, its owners, staff, volunteers or partners responsible for injuries or damages arising from normal museum use. This does not apply where the law does not allow liability to be waived, including gross negligence or intentional harm.
7. Responsibility for Damage
The signer agrees to pay for damage caused by themselves or their child when the damage results from intentional behavior, vandalism or failure to follow museum rules.
8. Photos & Videos
The digital form below requires the signer to choose whether the museum may use photos or video of the signer and/or child for marketing, social media, the website and promotional use without payment.
9. Agreement
By typing a full name and submitting the form, the signer confirms that they have read and understood the agreement, understand that certain legal rights are being waived, sign voluntarily and are the parent or legal guardian of the listed child or children.
